Insurance Coverage: It's advisable to contact your insurance company to determine the portion of the testing fee not covered by insurance, if any. Any remaining balance after insurance coverage becomes your responsibility.
Overpayment Refunds:If your initial payment exceeds the amount you're responsible for, you may be entitled to an overpayment refund. We hold these funds on your behalf.
Administrative Fees:To cover administrative expenses, we apply a monthly administrative fee of $10.00 for every month we hold any overpayment after the conclusion of the sleep test. However, if you contact us within 90 days of test conclusion to request your refund, we waive the administrative fee.
Failure to Request Refund:If you don't contact us within 90 days, accrued administrative fees will be applied to your account, and your credit balance will be adjusted accordingly. Your account will continue to be assessed the monthly administrative fee until your balance reaches $0.00 or you request a refund.
Voided Refund Checks:If we issue an overpayment refund check and it isn't cashed within 90 days, the check becomes void. In such cases, the administrative fee will be applied to your account, and your credit balance will be adjusted. We'll continue this process until your balance reaches $0.00 or you request another refund.
